Welcome to MotionWorks Physical Therapy! These Terms and Conditions (“Terms”) govern your use of our website www.motionworkspt.net (“Site”) and the services provided by our in-house team of licensed physical therapists.
By accessing or using our Site, you agree to be bound by these Terms. If you do not agree, please do not use our Site.
1. Services Provided
MotionWorks Physical Therapy connects users with licensed physical therapists from our clinic. Physical Therapy services may include but are not limited to evaluations, therapy sessions, and screenings, all booked through our Site or third-party scheduling platform, JaneApp.
2. Eligibility
To use our Site and connect to our services, you must be:
- At least 18 years old
*MotionWorks PT does treat children under 18 with consent from a guardian
- Residing in the United States
- Providing accurate and truthful personal and health information
3. Medical Disclaimer
The content on our Site is provided for informational purposes only and does not constitute medical advice, diagnosis, or treatment. Always consult directly with a licensed healthcare provider for any medical concerns.
Use of our website does not establish a patient-provider relationship until an official appointment is completed.
4. Appointments and Cancellations
Appointments are scheduled via a third-party booking platform, JaneApp. By booking, you agree to:
- Provide accurate personal and insurance information
- Arrive on time or notify us at least 24 hours in advance for cancellations
- Accept any applicable cancellation fees as stated in our policies
We reserve the right to reschedule or cancel appointments as needed.
5. Fees and Insurance
Service costs may vary based on your insurance coverage and the type of treatment provided. We provide a courtesy insurance verification prior to your initial appointment.
We accept Medicare and Blue Cross Blue Shield of Michigan insurance plans. Prior to your appointment, with your insurance information provided, we will be able to verify your insurance plan details and review it with you at time of service.
We also offer the option of cash-based payment plans and sessions.
Comprehensive Evaluation $160 for 60 minutes
Follow Up Treatment Session $120 for 60 minutes or $80 for 30 minutes

It is your responsibility to:
- Verify coverage with your insurance provider
- Pay any out-of-pocket expenses or co-pays as applicable
6. User Conduct
You agree not to:
- Use our Site for unlawful, harmful, or abusive purposes
- Attempt to interfere with or hack the Site or booking systems
- Impersonate any person or provide false information
We reserve the right to suspend or terminate access to users who violate these Terms.
7. Intellectual Property
All content on the Site—including logos, text, images, and software—is the property of MotionWorks Physical Therapy and protected by copyright and trademark laws. You may not reproduce or distribute any content without written permission.
8. Third-Party Services
We use a third-party provider to facilitate appointment bookings, JaneApp. While we choose our partners carefully, we are not responsible for the privacy policies or practices of third-party sites.

9. Limitation of Liability
To the fullest extent permitted by law, MotionWorks Physical Therapy is not liable for:
- Any indirect or consequential damages
- Inaccuracies or delays in services
